SOLO CLASS APPLICATION FORM pdf (this form includes a photo release form)

DUET-Trio-Ensemble App form pdf (you must also print off one photo release form per person)

BANDS or CHOIRS APPLICATION pdf

PHOTOGRAPH OR VIDEO RELEASE CONSENT FORM pdf (Please note that only one photo/video release form is required per person, even if that person has entered more than one class.)

Applications forms are formatted for LETTER sized paper.

Receipts will be issued only on request.  If you require a receipt please include a stamped, self-addressed envelope with your registration forms or arrange a pick-up time with the Registrar.

Please consult the syllabus for class numbers and fees.  Please read the Rules and Regulations document carefully before applying.
